This form outlines conditions and general provisions that apply to ARC grants. Not all are applicable to non-construction projects, however this form is required for all grant applications.

For Broadband as a Service (BaaS) Project Applicants Only. 

This form includes additional requirements and other provisions that apply only to BaaS projects. It must be completed and submitted along with all other required application forms and documentation material prior to ARC funding consideration.  

Note: This form must accompany ARC’s original MOU: “Memorandum of Understanding for ARC Projects”. Both MOUs must be submitted by BaaS Project Applicants.
